How to Enrol
To enrol as a patient, please complete our Enrolment Form. Once submitted, you can access subsidised primary healthcare services at Kaikōura Healthcare.
📍Important: In New Zealand, the Ministry of Health subsidises primary health care services, but you can only receive subsidised care from one medical practice of your choice. You can visit other doctors, but subsidies will only apply to your enrolled practice.

You will also need to complete a Health Questionnaire and bring it along with your Enrolment Form and two forms of identification.
We require ID, proof of eligibility and proof of entitlement for all new enrolling patients.
Accepted forms of ID and Proof of Eligibility (any one of the below options):
- NZ Passport
- NZ Birth Certificate (incl. Cook Island/Niue/Tokelau) with Photo ID (Driver’s License)
- NZ Birth Certificate (can only be accepted on its own for patients under 18 years of age)
- NZ Citizenship Certificate + Photo ID (International Passport, Driver’s License)
- Gold Card/Community Services Card + Photo ID (International Passport, Driver’s License)
- International Passport with copy of 2 years’ worth of consecutive Visa (Excludes Student Visa)
- Australian Passport and proof of commitment to stay in NZ for 2 years from arrival date i.e. Utilities bill, proof of NZ employment, letter from WINZ
Accepted forms of Proof of Entitlement
- Utilities bill (i.e. power/gas) with a Kaikōura or wider Kaikōura area address
- NZ Employment contract
We reserve the right to refuse enrolment to patients who:
- Cannot provide sufficient evidence to support their eligibility and entitlement to enrol
- Transfer providers on a regular basis
- Are verbally/physically abusive or threatening towards our team
Privacy & Data Protection
Kaikōura Healthcare complies with the Privacy Act 1993 and the Health Information Privacy Code 1994. This ensures the secure collection, use, storage, and disposal of your personal health information.
